Documents required for restoration of Vietnamese nationality

I was granted a foreign citizenship and gave up Vietnamese nationality. I want to apply to get Vietnamese nationality back, for myself and my children (10 - 12 years old) ?

Documents required for restoration of Vietnamese nationality of a minor child:

When a minor child restoring Vietnamese nationality together with his/her parents, copies of his/her birth certificate or other lawful papers proving father-child relationship, mother-child relationship shall be submitted. In case only either of the parents restoring Vietnamese nationality and the minor child living with that person is restoring Vietnamese nationality together with his/her father or mother, a written agreement of parents regarding the restoration of Vietnamese nationality for the child shall be submitted. Such written agreement must have full signatures of parents, the signatures are not required to be authenticated but the applicant for restoration of Vietnamese nationality for the child shall be responsible for the accuracy of the other’s signature.

In case a father or mother has passed away, has lost civil act capacity or has a limited civil act capacity, the written agreement shall be replaced by papers proving the father or the mother has died, has lost or has a limited civil act capacity.
